This is an arrangement from the Sega genesis game Earthworm Jim. This particular song shows up in the level Buttville, and is called "The Descent". This arrangement is modeled off the original genesis game, not the Sega CD remix done by Tommy Tallarico, which is pretty damn cool in its own right.
